  • Above 1 month: 1-2.5 mg of IV every 8 hours
  • 0-4 weeks with birth weight less 1200 g: give 2.5 mg of IV after every 18-24 hours
  • 1-4 weeks with birth weight above 2000 g: give 2.5 mg of IM after every 8 hours.
